Discussion on the Application of Hinge in Knee Prosthesis_Hinge Knowledge

Severe knee instability can be caused by conditions such as valgus and flexion deformities, collateral ligament rupture or loss of function, large bone defects in the distal femur and proximal tibia, and incomplete coverage of soft tissues. In order to address these issues, certain types of knee prostheses are required, such as the hinged knee joint prosthesis.

The rotating hinge knee prosthesis (RHK) is the third generation of hinged knee prosthesis that is widely used in clinical practice. It includes prostheses such as the S-ROM Modular Mobil-Bearing Hinge Prosthesis, Finn Knee, and Link PK. These prostheses have overcome the limitations of earlier designs in terms of stress conduction, materials, and other factors, and have shown better clinical results with reduced incidence of complications.

However, there have been conflicting reports in the literature regarding the clinical efficacy and complications of RHK. Therefore, this study aims to systematically analyze relevant literature to determine the overall complications and occurrence of major complications after RHK.

The study conducted a comprehensive literature search in several databases and applied inclusion criteria to select relevant studies. The quality of the included literature was evaluated, and statistical analysis was performed to calculate the rates of total complications and specific complications such as periprosthetic infection, aseptic loosening of prostheses, periprosthetic fractures, and patella-related complications.

The results showed that the overall complication rate after RHK was 23.6%, with the most common complications being periprosthetic infection (6.5%), aseptic loosening of prostheses (2.9%), periprosthetic fractures (3.8%), and patella-related complications (3.8%).

It is important to note that these findings are based on a limited number of studies and there is a possibility of biased reporting in the literature. Therefore, further high-quality research is needed to provide more comprehensive information on the occurrence of complications after RHK and to identify the causes and influencing factors.

In conclusion, understanding the postoperative complications of RHK can help clinicians make informed decisions and provide better care for patients. Further research is needed to improve the design of RHK, refine clinical indications, and enhance postoperative rehabilitation in order to maximize the benefits for patients with knee conditions.
